The Data Protection Act 1998 is an Act of Parliament implemented to ensure the confidentiality of information held by any organisation. This includes your personal and medical information.

You do have the right to keep all personal and medical data that is held by the surgery private to third party organisations (this includes hospitals, pharmacies etc.), you will need to fill out an opt out slip. These can be found at the reception desk in the surgery or can be printed by using the "Data Sharing Opt-out" link below. This will need to be handed to a member of staff at the surgery.
